- Download the Claim Form here
- Fax and post the completed form and claim supporting documents to the fax number and address as stated in the form.
- Please note the Company shall not be liable for any claim of which no notice has been received by the Company within 30 days after the occurrence of the event.

Important Note:If your insurance policy was purchased on or before 16 Feb 2009, you should direct your claim to the previous insurer, namely AIG General Insurance (Malaysia) Bhd at the address given in the Certificate of Insurance or call them at telephone number : + 603 2772 5622
